Wearing a sacramental, such as a scapular, medal, etc., while committing a sin would not normally be an additional sin to be confessed. It would only be an additional sin if the sinner directly intended to blaspheme or commit sacrilege in this way, or did it with the intentional purpose of "putting the Lord to the Test."

Simply sinning while wearing the sacramental (giving it no thought, or not bothering to remove it) would not entail an additional sin.

Is it a sin to comitt a mortal sin while wearing a sacramental such as a medal of Our Lady or a Scapular?

Yes it is still a sin. Now is it a double sin? shouldn't be.

the only way it could be 2 mortal sins, is if you willed, or you fully consented to BOTH being mortal. Otherwise commiting one mortal sin with a sacramental on, if that was a mortal sin in the first place which I'm not sure about, and not being aware of the fact that it would be one if you willed it. Wow... make sense??

Wait will god forgive our sins if we sin while wearing a scapular

Are you repentant and genuinely desire to not sin again?  Will you take steps to amend your ways in order to better avoid  committing the same sin?

If you answered, yes, then God will forgive any sin.   Go to confession and thank Him for his mercy, and ask for the grace to avoid scrupulosity.
