Nihil Obstat
Yeah, take it slow I suppose. Definitely talking to the priest would be the best option, because he may have some suggestions, or even might have dealt with a similar situation in the past. I don't want to chuck him into the deep end! It would be good to be able to receive though, so I will try and speak with him.
Aloysius
if the condition is serious enough they would likely be willing to accommodate you with a spoonful or a small chalice of the Precious Blood, and they might prefer that to communion on the hand (depending on the priest, if it's someone who regularly does the Novus Ordo also, he'd probably be fine with the hand if your condition seemed to make it necessary) but anyway they're very experienced with distributing communion to the mouth so I think also if you're maybe not able to stick your tongue out properly or something I'm sure they've placed it straight into an open mouth before. it's not too hard to do without having to get your finger in there. depending on what it is, it is probably a good idea to speak to the priest beforehand (maybe check for an email or phone number of the church as you never know how easy it will be to find Father before mass. Thanks for this :) My condition's not serious, it just makes some facial movements (like opening my mouth wide or sticking out/moving my tongue) a bit more difficult. The main issue is I can't stick my tongue out that far and I know I may have an issue with swallowing if the host is placed directly on my tongue. I'll see if I can get in touch with the priest.
