The story behind those scratches comes from a tale that talks about the Hugonauts, and how, because of their iconclasm, destroyed icon after icon.They came to this one, which is said to have been written by the hand of St. Luke the Evangelist. One Hugonaut stabbed the Theotokos in the face, and it began to bleed. Well, don't quote me on it (like I quoted you above for example), but what I understand from it is that the throne of Peter is one of pain and suffering, as Christ has suffered, so does the pope in our lifetime. So yes, those are thorns representing the deep seeded dificulties and pains of taking on the office of Peter.
