It's called "Jesus my Lord, my God, my All." I heard it at Mass one day for the first time, and it so happend that on my Birthday which was on a Sunday this year, it was also a Hymn used that day at Mass. I wasn't able to find many good versions on YouTube, but here is a good violin version for a couple of verses of the Hymn (They called it here "Sweet Sacrament.")
