One example would be:

Obviously one must actually mean the words one is saying, or it wouldn't really be an act of perfect contrition.

BTW, it's super important to get in the habit of making heart-felt acts of perfect contrition, because you never know when you might really need to be able to make one.

At any time, but especially after you've committed a mortal sin (or any sin for that matter) and want to be forgiven.

That said, according to the [i]CCC[/i], you have to have the intention of going to confession as soon as you can if you want to be forgiven of a mortal sin by an act of perfect contrition.

I don't think that one can absolutely, positively, know for sure if one's contrition is perfect, but I would say that one can have reasonable certainty.

If you were walking to confession and got shot by a mugger, and managed to make an act of contrition, there's pretty good certainty. :D Just as a for instance.

One's act of contrition would have to be perfect (i.e. motivated by love of God) though. :)

Yes. :)

I believe true confession is between you and God the priest is only a medium. If you are truly sorry then you have Gods forgiveness and in any event Jesus came to earth to save you.

You have to be truly sorry out of love for God and not just out of fear of hell in order to be forgiven of mortal sin extra-sacramentally.

Also, in order to be forgiven of mortal sin extra-sacramentally one must have the intention of going to confession.
